Licensed Practical Nurse, LPN - Home Health - Devils Lake

Location:

Altru Clinic, Devils Lake, ND 58301. Pay Range: $20.75 – $31.12 per hour.

Summary:

The Licensed Practical Nurse (LPN) performs assessments, records patient histories, and coordinates admission and discharge plans. The LPN provides patient care, personal assistance, and documentation while collaborating with the multidisciplinary team.

• Assist the registered nurse or licensed practitioner in identifying patient needs, priorities, and goals based on individual demographics.
• Gather, observe, record, and communicate patient responses to nursing interventions.
• Modify the plan of care in collaboration with a licensed practitioner.
• Execute department‑specific functions in accordance with standards of care, policies, and procedures.
• Coordinate and delegate nursing interventions to qualified personnel.
• Identify early warning signs of patient condition changes and report appropriately.
• Implement nursing interventions and prescribed medical regimens safely and document care accurately.
• Collaborate and communicate timely patient information with patients, families, and the multidisciplinary team.
• Report patient pain symptoms promptly following standard operating procedures.
• Participate in health education approved by a licensed practitioner.
• Assist in discharge planning, education, and counseling to optimize patient and family understanding.
• Perform other duties as assigned to meet departmental needs.

• Licensed Practical Nurse (LPN) – North Dakota Board of Nursing or Compact State Agreement – prior to start date.
• Licensed Practical Nurse (LPN) – Minnesota Board of Nursing – within 12 months of start date.
• Driver’s license with acceptable driving record – current state licensed – prior to start date.
• Basic Life Support (BLS) – American Heart Association – within 90 days of start date.

• Proficient reading, writing, and speaking in English to communicate effectively with patients, families, and team members.
