Position Overview

The Nursing Assistant (NA) serves as caregiver to the resident during the scheduled work period in long‑term care. Provides resident‑centered nursing care and daily living assistance under the supervision of a registered nurse (RN). Delivers age‑appropriate care related to the physical and psychological needs of the resident as per the care plan and is accountable for infection prevention and control policies and Personal Protective Equipment use.

• Assist with basic health care needs, including bathing, toileting, grooming, dressing, recording vital signs, and providing psychosocial support
• Assist residents in transferring, repositioning, and walking using correct transfer techniques and equipment; provide range‑of‑motion and passive exercises
• Complete tasks related to food preparation and dining, ensuring a positive dining experience
• Assist residents with meals and snacks, provide water and supplements as appropriate
• Assist providers and nursing staff with procedures when needed
• Document resident interactions and outcomes related to care, behavior, activity, and diet
• Communicate resident’s changing condition and care‑related concerns to the charge nurse
• Collect non‑invasive body fluid specimens and gather vital signs
• Provide housekeeping and laundry services to maintain living areas
• Assist with wellness and recreational programs and encourage resident participation
• May participate in departmental or organizational committees and task forces as needed

• High school graduate or GED preferred (not required); minimum age 16
• Prior clinical or nursing assistant experience in long‑term care preferred
• Certified Nursing Assistant (CNA) certification with the State Board of Nursing or state‑certified CNA within four months of start date; enrollment in a training program or endorsement program acceptable
• CNA training and certification required and provided by the facility
• Complete yearly department‑specific competency testing and maintain a record of continuing education credits; at least 12 hours of continuing education annually
• Maintain all department‑specific required certifications; when applicable, be certified and registered with the Unlicensed Assistive Person (UAP) Registry
• If driving responsibilities apply, may be classified as a Sanford Category II Driver; requires a motor vehicle report, a valid driver’s license, and medical qualification per Sanford Driving Policy
