Roles & Responsibilities   Provide skilled physical therapy evaluations and develop individualized home health treatment plans
Deliver hands-on therapy and progressive interventions to improve mobility, safety, and function in the home
Educate patients and caregivers on safe transfers, fall prevention, and home exercise programs
Document patient progress accurately and timely in the Home Care Home Base charting system
Collaborate with interdisciplinary team members to coordinate care and optimize outcomes
Participate in orientation and required training to maintain clinical competencies
Qualifications   Valid Physical Therapist license (ability to submit license required)
Minimum 1 year of clinical experience (home health experience preferred)
Current BLS certification (required)
Comfortable working independently in patients' homes and managing caseloads
Willingness to complete 12 hours of orientation as part of onboarding
Knowledge & Skills   Strong knowledge of home health therapy principles and home safety assessment
Skilled in mobility, gait training, balance interventions, and fall prevention strategies
Proficient documentation skills, experience with Home Care Home Base is a plus
Excellent patient and caregiver education and communication skills
Ability to collaborate effectively with nurses, case managers, and other clinicians
