Shift: 8H Days Days+Call - M-F 8a-5p + On-Call rotation evenings/weekends, holiday coverage required

Job Description:

Responsible for initial and ongoing nursing assessments and care planning for home health clients. Conduct physical assessments, administer medications, develop care plans, and ensure appropriate discharge planning.

Responsibilities:

• Initial assessment of client and family to determine home health needs, complete physical assessment and history.
• Regularly re-evaluates client nursing needs and uses health assessment data to determine nursing diagnosis.
• Initiates and revises plan of care based on client status and needs, includes client/family in planning process.
• Administers medications and treatments as prescribed, provides health care instructions to clients, and prepares clinical notes.
• Acts as Case Manager when assigned, coordinates client care, and communicates with physicians regarding client needs.
• Instructs, supervises, and evaluates home health aide care provided.

Requirements:

• Home Health & Hospice experience required.
• Must have current Driver's License & Auto Insurance.
• BLS Certification required.
