Job SummaryFully accredited by the Commission on Cancer, Geisinger’s state-of-the-art cancer center in Dickson City brings all aspects of cancer care together. The Dickson City facility offers radiation oncology, a pharmacy, laboratory services and palliative medicine, along with patient amenities. The RN Coordinator Cancer Care serves to assess patient needs, plan and provide nursing care, and coordinate follow-up care for patients receiving treatment in an outpatient Hematology/Oncology setting.
The role focuses on care navigation throughout the continuum of care, including patient education, triage and care coordination.
- Coordinate the treatment plan for Hematology and Oncology patients through the continuum of care.
- Provide ongoing patient care and education for patients in the adult Hem/Onc Department.
- Serve as a resource for patients and families regarding diagnosis and medical plan of care as designated by the physician.
- Act as a patient liaison between the patient and care team, addressing issues/obstacles/changes to the plan of care.
- Perform pre-visit RN triage to facilitate first-visit decisions and initiate treatment planning for Hematology/Oncology care.
- Assist with supportive care referrals (Palliative care, Psychology, Social Work).
- Complete and document initial chemotherapy education session in EPIC as required by regulatory bodies; work toward OCN certification after required hours are completed.
- Participate in performance improvement activities and assist providers with completion of tasks.
- Assist with orientation of new personnel to ensure proper support for patients and providers.
- Execute and document ongoing patient communication; instruct patients to ensure understanding of medical condition and treatment instructions.
- Perform and assist with triaging telephone calls concerning health issues and provide telephone management advice as liaison to physician recommendations.
- Communicate Lab, Radiology and other testing results to patients via telephone and EMR notifications.
- Graduate from Specialty Training Program-Nursing (Required)
- Bachelor's Degree-Nursing (Preferred)
- Minimum of 2 Years-Nursing (Required)
- Licensed Registered Nurse (Pennsylvania) - RN;
Basic Life Support Certification;
Chemotherapy and Biotherapy certification within 3 months of hire;
Oncology Certified Nurse (ONCC) after completion of required hours
