Tennessee Quality Care is seeking a PRN Physical Therapist to join our Home Health team in Camden, TN and surrounding counties. You will provide one-on-one skilled PT services in patients’ homes, helping them regain mobility and independence.
Responsibilities include evaluating patients, creating individualized plans, educating patients and families, and coordinating with the care team. CPR-certified, APCP-licensed, and valid driver’s license required;
Home Health experience preferred.
