Responsibilities:

• Coordinate the collection, validation, and submission of Minimum Data Set (MDS) assessments, care plans, and resident information in a timely and accurate manner

• Collaborate with healthcare providers, therapists, and other team members to ensure comprehensive care plans that meet each resident's unique needs

• Review and analyze MDS data to identify trends, patterns, and areas for improvement, and implement changes as needed

• Develop and maintain effective relationships with residents, families, and healthcare providers to ensure excellent customer service and communication

• Participate in quality improvement initiatives and regulatory compliance efforts to maintain our high standard of care

• Maintain accurate and detailed records of all MDS assessments, care plans, and resident information
