Job Summary and Qualifications
The Sepsis Coordinator facilitates the development, implementation and ongoing adherence of sepsis program protocols and therapies. Ensures the protocols and therapies become the standard of care followed within the facility. Collaborates with medical and hospital staff to provide multidisciplinary care to patients with a diagnosis of sepsis. Rounds with physicians on patients with a diagnosis of sepsis and reviews clinical data.
Provides education and clinical expertise to care team, patients and families. Collaborates with physicians and staff in an effort to decrease mortality and length of stay. Identifies and integrates patient learning needs, abilities and readiness into the Plan of Care. Works in collaboration with the nursing team to develop and implement individualized teaching plans for patients and families regarding sepsis diagnosis.
Collects and analyzes sepsis program patient data to assist with documentation, clinical care and improved outcomes. Participates in rounds and proactively responds to patient needs to improve patient outcomes and positively impact overall patient experience.
- Review all positive screens on a daily basis and collaborate with primary nurse and physician. Able to navigate Meditech with confidence and retrieve patient care information.
- Demonstrate responsibility and accountability to assure early goal‑directed therapy is initiated.
- Provide leadership to promote teamwork; work with multidisciplinary groups and facilitate problem‑solving tasks, using critical thinking techniques.
- Initiate and guide staff in appropriate interventions associated with sepsis protocols.
- Able to articulate the NPSG meaning and measuring process; use critical thinking and effective communication with other healthcare providers and departments.
- Provide regular in‑services to nursing staff and ensure understanding. Participate in hospital orientations as needed regarding sepsis education.
- Collect and present abstracted data for analysis purposes. Organize sepsis meetings to assure dissemination of relevant information. Attend division meetings and other activities as required.
- Communicate daily to answer and/or educate nursing staff. Responsible to stay abreast of teamroom updates and files as required.
- Round with physicians as necessary on sepsis patients; provide guidance as required.
- Identify role as a team member and works efficiently to achieve goals of team, department and hospital. Demonstrate professional behavior in actions and consistently portray a positive attitude in response to work changes.
- Demonstrate courtesy and caring through excellent customer service.
- Communicate clearly, directly and professionally. Listen and respond with empathy.
- Actively attend all staff meetings and keep current on departmental communications and information including patient safety and quality improvements.
- Timely completion/submittal as per hospital policy for Licensure, Certifications, annual TB and department‑specific competencies as required by job description.
- Follow the policies for attendance, punctuality, time‑keeping, meal breaks, usage of cell phones, dress code and confidentiality of information.
- Demonstrate adherence to the Code of Conduct and reflect Horizon’s values and commitment to the Code of Conduct.
- Bachelor’s Degree in Nursing required;
Master’s Degree preferred. - Current licensure as a Registered Nurse (RN) in the state of Tennessee or a compact state required.
- 3-5 years of prior clinical experience preferred.
- Experience with personal computers using spreadsheets and word processing required.
- BLS required.
