You will lead person-centered planning, help people discover strengths, and coordinate services across Day and Residential programs, Medicaid/MCOs, SSI/SSDI, SNAP, VR, and health supports. Requirements:
Bachelor's degree and 6+ months experience with IDD or equivalent; valid driver's license and clean driving record; ability to pass background check and drug screening.
