• Active, unencumbered LPN/LVN license in the state where you will practice
• Compact licenses must be transferred to your state of residence within 90 days
• Current CPR certification with hands‑on component (the company can assist after hire)
• Tuberculosis skin test, current within the last 12 months
• At least 6 months hands‑on experience; new graduates are not accepted
• Reliable transportation required
