The Change Realization Consultant is an individual contributor skilled in change leadership and partners with Senior Change Realization Consultants and the AVP of Change Realization in facilitating and sustaining change. This position is responsible for planning, implementing, and overseeing change initiatives within the organization, focusing on the people side of change—ensuring adoption and minimizing resistance to changes in processes, systems, technologies, and structures.
The Change Realization Consultant is responsible for supporting fellow change colleagues by organizing events such as change implementation planning sessions, education, and the enterprise-wide community of practice.

The Change Realization Consultant is an individual contributor skilled in change leadership and partners with Senior Change Realization Consultants and the AVP of Change Realization in facilitating and sustaining change. This position is responsible for planning, implementing, and overseeing change initiatives within the organization, focusing on the people side of change—ensuring adoption and minimizing resistance to changes in processes, systems, technologies, and structures.
The consultant supports fellow change colleagues by organizing events such as change implementation planning sessions, education, and the enterprise-wide community of practice.
- Organize and support change initiatives such as education and change implementation planning sessions; co‑facilitate education programs.
- Integrate change realization activities into project plans and coordinate and support change function project plan activities.
- Create deliverables for communications, roadmaps, education, and resistance management; develop a calendar of change events.
- Research best practices and support the development of change leadership materials for organization‑wide programs.
- Track feedback and success metrics, report on adoption, utilization, and proficiency; maintain records of change initiatives, leaders, and agents.
- Model Change Agent behaviors to instill a change culture, build strong relationships and communications, and position process change work as a key enabler of strategy to execution efforts.
- Bachelor’s degree in Healthcare Administration, Education, or a related field.
- At least 2 years of experience leading and supporting teams through change initiatives or within an organization development field, including learning and organization development.
- Familiarity with industry standard change management methodologies such as CAP, Prosci, SEF, etc.; change management certification preferred.
- Master’s degree in Healthcare Administration, Education, or a related field.
- Strong communication and analytical skills.
- Active listening and relationship‑building skills.
- Experience with large‑scale change efforts.
- Ability to influence and lead change across diverse stakeholder groups.
- Keen attention to detail and advanced organizational and time‑management skills; ability to develop and manage a project plan incorporating change management principles.
- Designs evaluation methodology and tools to measure program effectiveness and provide reports.
- Excellent project management and organizational abilities.
- Proficiency with Microsoft 365.
- ICGB – Certified Lean Six Sigma Green Belt – International Association for Six Sigma Certification.
- CCMP – Certified Change Management Professional – PROSCI.
- GE Master Change Agent – General Electric.
Lift and carry up to 25 lbs. Frequent sitting/standing, frequent keyboard use. Patient care providers may be required to perform activities specific to their role, including kneeling, bending, squatting, and performing CPR.
