Case Manager
- Supportive Housing Tuesday-Saturday 12:00pm-8:00pm.

POSITION SUMMARY/FUNCTIONResponsible for providing care coordination / case management services to those with mental illness, substance use and/or developmental disabilities. Conduct interviews and assist in accessing appropriate community services. Maintain appropriate patient records and tracking of patients between agencies. Some travel may be required within the catchment area.

ESSENTIAL DUTIES/RESPONSIBLITIES/FUNCTIONS/TASKS:

Provide initial care coordination/case management services and develop written service plans for each patient.

Conduct face to face interviews (where appropriate) with patients and/or patient representative to assess individual needs, supports, goals, etc.

Information gathered may include but not limited to: identifying information, community living, employment, physical needs, health, social history, housing and physical environment, socialization and recreation.

Identify appropriate community service needs of patients and determine appropriate community resources to meet those needs.

Involve the patient and/or family in the development of the written service plan and conduct in a timely manner.

Effectively implement service plans for patients.

Coordinate service delivery to patients among appropriate agencies as specified in the service plan.

Provide family support and/or basic living skills as needed.

Conduct periodic assessment and modification of service plan.

Advocate on behalf of patient and target population as a whole regarding social, economic, and residential requirements.

Conduct home visits.

Provide transportation only when it is a necessity. Ensure all community resources such as family, friends, public transportation and special vehicles are utilized to meet the needs of patient. If transporting clients in personal vehicle, must maintain additional liability insurance as required.

Coordinate care for patients between agencies and monitor/follow up with the patient and agency regarding this care.

Close cases as appropriate.

Meet required productivity level of 80% direct service time.

Assist with maintaining patient records in a professional manner and in accordance with all Spectra Care policies and procedures, Alabama Administrative Code, CMS, and all other applicable regulating and contractual entities.

Ensure privacy and confidentiality of all patient records in accordance with HIPAA, 42 CFR and all other applicable state or federal laws.

Ensure all written documentation is completed accurately and in a timely manner.

Ensure that all records are staffed within standard time frames, with service plan revised as needed and in accordance with regulatory requirements.

Ensure that all progress notes are completed promptly, written in a professional manner and in accordance with policies and procedures.

Ensure that goals are appropriate, specific, and the focus of treatment.

Ensure that all patient information is kept up to date.

Ensure patient release of information forms are signed in a timely manner and maintained within the chart.

Administrative and other related duties as assigned to include but not limited to:

Serve as a liaison and advocate for Spectra Care patients with community agencies and the public.

Assist with ensuring the care coordination/case management services remain within budget parameters.

Actively participates in committees as required.

Completes assigned tasks in a timely manner.

Works in a cooperative manner with other Spectra Care employees.

Receive and respond to inquiries promptly and courteously.

Must perform all duties with the highest standard of ethics.

Attends regular workshops, trainings, in-services, and/or administrative meetings as appropriate.

Maintain travel records accurately and submitted within the appropriate timeframe.

Ensure necessary documentation is completed and up to date before accrued leave is taken.

Adhere to Dress Code as set forth in the Spectra Care policies and procedures.

Follow guidelines of Spectra Care Personnel Policies, Spectra Care Clinical Policies, Alabama Department of Mental Health Administrative Code, CMS, and all other applicable regulating and contractual entities.

Performs duties independently and/or with minimal supervision.

Functions within professional boundaries with patients and others.

Works well with other staff and patients to ensure safety of program, patients, and other staff.

Functions and performs well in stressful environments and manages work related stress to ensure client safety.
