Early Intervention​/Service Coordinator

This position is responsible for coordinating the agency's early intervention services and delivery of special instruction to children served.

• Processes child referrals for early intervention services; reviews applications and applies testing criteria to determine eligibility; limits caseload to children determined eligible; ensures that appropriate Release of Information forms are signed.
• Develops Individual Family Support Plans (IFSPs) for each infant/toddler/child; conducts face-to- face Routine Base interviews with child and family to assesses the child's needs; identifies appropriate community service needs; involves parents and other professionals in the development of IFSPs; maintains related records in accordance with Alabama Department of Mental Health, Medicaid, Alabama Department of Rehabilitation, and agency guidelines.
• Implements IFSPs effectively; reviews and revises plans as needed; coordinates services among appropriate agencies, including physical therapy, occupational therapy, special instruction, and speech therapy; schedules, confirms, and reschedules appointments; monitors service session to ensure compliance with standards; encourages family involvement and advocates on behalf of children and their families; conducts periodic plan assessments and modifications; maintains specific measurement criteria; maintains inter-agency tracking system for children/families served;
  
  maintains required case management files; closes or transfer cases as appropriate.
• Plans, develops, and implements the programs and activities of the curriculum; prepares IFSPs using developmentally sequenced programs that are appropriate to the child's/family's needs; identifies and prepares materials and supplies for various goals/objectives or activities; assists with assessments and appraisals; delivers special instruction; teaches child/family members basic skills in identified areas of need.
• Shares information with the public, individuals, and referral sources concerning agency programs and services.
• Maintains records of each child served, including procedural safeguards, releases, medical records, IFSPs, case management notes, evaluation and assessment results, eligibility determination reports, progress notes, etc.
• Maintains certification in CPR, First Aid, and CPI.
• Performs other duties and responsibilities deemed necessary; not otherwise specified.

The Associate Executive Director of Programs and Operations supervises and assigns work in terms of very general instructions. The supervisor spot-checks completed work for compliance with procedures and the nature and propriety of the final results.

Guidelines include agency policies, Alabama Department of Mental Health guidelines, Medicaid guidelines, and Alabama Department of Rehabilitation guidelines. These guidelines require judgment, selection, and interpretation in application.

• The work consists of varied early intervention cases management duties. The unique needs of each child contribute to the complexity of the position.
