Gentiva Hospice seeks an After Hours / On-Call Registered Nurse to respond to patient needs during evenings, nights, weekends, and holidays, providing direct care and symptom management in patients' homes or facilities. You will act as the liaison among patients, families, physicians, and the hospice team to ensure seamless continuity of care.
The role requires an active RN license, at least 1 year of nursing experience, excellent clinical judgment, and strong EMR documentation skills.
