Licensed Practical Nurse

We are seeking a Practical Nurse to provide medical services for individuals with intellectual and developmental disabilities. This role involves supporting individual care plans, monitoring health indicators, and documenting care.

• Must successfully complete background and drug screening.
• Valid driver's license with required endorsements and at least 3 years of driving experience.
• Basic computer skills.
• Current and valid license for Practical Nurse.
• Experience working with people with intellectual/developmental disabilities preferred.

• Assist in providing basic healthcare services according to the individual's plan, ensuring optimal health and wellbeing.
• Perform tasks as delegated, such as blood pressure monitoring, weight measurement, and other assigned duties.
• Ensure all documentation is accurate and complete according to regulations.
• Organize and maintain cleanliness of supplies and equipment, restocking inventory.
• Collaborate with Direct Support Professionals to meet individual needs.
• Immediately communicate any changes in medical or behavioral status to supervisor.
• Perform all other duties as assigned.
