Phoenix Senior Living is seeking a dedicated LPN to join our Healthcare & Medical Services team. In this role, you'll deliver compassionate resident care, administer medications, monitor health status, and support care plans in collaboration with an interdisciplinary team. Responsibilities
- Provide direct nursing care to residents, including assessments, treatments, and monitoring of health status
- Administer medications and treatments in accordance with physician orders and state regulations
- Document care, observations, and resident responses accurately and timely in electronic medical records or charts
- Assist in developing, implementing, and updating individualized care plans in collaboration with RNs and physicians
- Communicate changes in resident condition to supervisors, physicians, and family members as appropriate
- Support and guide care staff to ensure safe, high-quality resident care and adherence to protocols
- Promote resident dignity, rights, and independence while providing assistance with health-related needs
- Follow all infection control, safety, and regulatory requirements within the community
- Participate in shift hand-offs, care conferences, and interdisciplinary team meetings
- Contribute to a positive, family-like culture aligned with Phoenix Senior Living's values of integrity, quality care, and excellence
