Part Time - Licensed Practical Nurse

We are seeking a dedicated Licensed Practical Nurse (LPN) to join our team, reporting directly to the Wellness Director.



Key Responsibilities Include

• Monitoring residents' health and wellbeing through assessments and personalized service plans.
• Administering medications and maintaining clinical quality assurance.
• Communicating with residents, families, and support services regarding care needs.
• Assisting with monthly wellness visits, vital signs, and documentation of clinical changes.
• Supporting direct resident care as needed and updating care plans accordingly.
• Identifying changes in resident conditions that may impact their care level or housing arrangements.

• Active LPN or RN license.
• Minimum of one year experience working with elders or disabled individuals (preferred).
• Leadership capabilities and knowledge of assisted living, dementia care, or skilled nursing (preferred).
• Flexibility to work evenings, weekends, and peak service hours.
