Stephen Center has served homeless and low-income individuals in Omaha since 1984 with the mission to partner with the community, families and individuals to overcome homelessness, addiction and poverty. The Pettigrew Emergency Shelter and Permanent Supportive Housing offerings provide safe and secure housing while individuals and families complete an assessment of permanent housing options. All Stephen Center residents have access to dual diagnosis support services via the HERO addiction/recovery program.

100% of Stephen Center shelter supportive housing participants are living in poverty. Housing transition support identifies poverty barriers that may impact one’s ability to achieve or maintain permanent housing through case management, life skills, and behavioral health services. Stephen Center serves approximately 250 people daily in all programs.

Purpose: The Street Outreach Case Manager plays a critical role in Stephen Center's mission to provide support, resources, and pathways to stability for individuals experiencing homelessness. This position focuses on building trust and relationships with unsheltered individuals while working collaboratively with other street outreach teams and Threshold Continuum of Care (CoC) serving Douglas, Sarpy, and Pottawattamie counties. Additionally, the Street Outreach Case Manager will provide case management support to Stephen Center shelter clients as needed, offering continuity of care to both sheltered and unsheltered individuals following Threshold and Stephen Center policy and procedures.

Key Responsibilities:

  • Conduct proactive outreach to individuals and families experiencing homelessness in the metro area, including encampments, public spaces, and other locations where unsheltered individuals may reside.
  • Build trusting relationships with clients to assess their needs, provide support, and facilitate access to resources.
  • Collaborate with local street outreach teams, including Threshold, to ensure coordinated and effective services.
  • Maintain comprehensive knowledge of community resources not limited to, but including housing programs, mental health services, addiction recovery support, and medical care.
  • Assist clients with accessing emergency shelter, long-term housing options, and other support programs.
  • Develop and monitor individualized case management plans, including goal setting and progress tracking.
  • Provide case management services to Stephen Center shelter clients, addressing their specific needs and goals for stability.
  • Support individuals who are not currently checked into a shelter by connecting them to available resources and programs.
  • Provide crisis intervention and de-escalation as needed.
  • Maintain accurate and timely documentation in compliance with organizational and funder standards.
  • Participate in community meetings, trainings, and collaborations to represent Stephen Center and improve service delivery.
  • Uphold the values of dignity, respect, and trauma-informed care in all interactions.
  • Other duties as assigned
  • Maintain Clarity license
  • Collect and enter data within 24 hours of service provided to client.

Position Requirements

  • Bachelor’s degree in social work, human services, psychology, or a related field; equivalent experience may be considered.
  • Experience working with individuals experiencing homelessness, addiction, or mental health challenges preferred.
  • Strong interpersonal, communication, and organizational skills.
  • Demonstrated ability to build rapport with diverse populations, including individuals in crisis.
  • Knowledge of community resources and referral processes in the Omaha area is a plus.
  • Valid driver’s license and reliable transportation required.

Working Conditions

  • The role requires working in outdoor environments and visiting unsheltered locations in various weather conditions.
  • May involve occasional evening and weekend hours to meet client needs.
