Health Connect America, Inc. seeks an Independent Contractor - Family Support Services professional to provide in home and community based support to children, adolescents, and families. Responsibilities include implementing strengths based family support plans, coaching caregivers in behavior management and daily living skills, coordinating with therapists and case managers, documenting services, and linking clients to community resources. Contractors collaborate within a multidisciplinary team, uphold ethical and trauma informed practices, and maintain flexible schedules while supporting client goals, safety, and overall well being across diverse communities.

Responsibilities

  • Provide in-home and community-based family support services to children, adolescents, and caregivers.
  • Implement strengths-based service and family support plans in collaboration with clinical teams.
  • Coach parents and caregivers in behavior management, communication, and daily living skills.
  • Coordinate care and communicate regularly with therapists, case managers, schools, and community partners.
  • Document all contacts, services, and progress notes accurately and on time.
  • Identify and connect families with appropriate community resources and supports.
  • Support safety planning and use basic crisis de-escalation techniques when needed.
  • Maintain professional boundaries, confidentiality, and compliance with ethical and regulatory standards.
  • Participate in multidisciplinary team meetings, supervision, and ongoing trainings.
  • Manage a flexible schedule to meet families in their homes and communities as needed.
