This role as Health/Human Service Case Manager I supports the unit that handles applications received through the ASSIST on‑line application tool and from the Federal Healthcare Marketplace. You will determine initial eligibility and continued eligibility for the following needs‑based benefits issued by the Division:
Food Supplement Benefits, Medicaid, S‑CHIP and Subsidized Child Care. In addition, you will make referrals to other agencies, such as Child Support, Children and Family Services, and food closets, to assist clients in obtaining benefits we do not provide.

This class is distinguished from a licensed Social Worker as employees do not perform the following duties: diagnose individuals for mental, emotional, behavioral, addictive, and/or developmental disorders and disabilities; perform related client assessment or evaluation; provide psychotherapeutic treatment; or develop and implement assessment‑based treatment plans. An employee in this class may participate as a member of a multi‑disciplinary team to implement behavioral health services or treatment plans in accordance with Delaware Code, Title 24.
- Interviews clients, family members, and others to determine eligibility and connect clients with health and human services.
- Develops a service plan to aid and assist a client in receiving care and assistance needed in collaboration with hospitals, clinics, agencies, and other appropriate resources (Service plans may include job training and employability, housing, general public assistance, in‑home services and supports or home‑delivered meals, de‑escalation techniques, peer services, or skill development).
- Evaluates information derived from client interviews, case history, and other pertinent sources and refers to proper channels for review.
- Provides guidance and relevant information to clients and families prior to receiving requested service and assistance.
- Coordinates with other agencies, hospitals, clinics, and resources to expedite the process on behalf of the client.
- Prepares case histories and reports and maintains records and chronological logs of case activities.
- Uses automated information systems and computer software to research, review, evaluate, and report on health/human service data.
- Communicates and interacts effectively with people across cultures, ranges of ability, genders, ethnicities, ages, and races.
- Performs other related duties as required.
Applicants must have education, training and/or experience demonstrating competence in each of the following areas:
- Possession of an Associate's degree or higher in Behavioral or Social Science or related field.
