Case Manager – Children’s Behavioral Health Program

Location:

Kent County, Delaware

Earn $20.67 per hour.

• Coordinate resources and information pertinent to client needs.
• Effectively and efficiently document all coordination of resources.
• Coordinate referrals for client specific discharge planning; ensuring that the family actively participates in creating a multi-faceted discharge plan that represents the family’s choice for service providers and community resources.
• Monitor record keeping, compliance and authorized units remaining, ensuring that client charts are up-to-date and include all necessary documentation, including documentation of significant incidents within 24 hours as needed.
• Monitor and assess documents for safety.
• Complete necessary case documentation that complies with the standards set by the company, county MHDS/CASSP, OMHSAS, and other licensing, regulatory and payor organizations.
• Deliver case sessions primarily in the community (client homes, schools, etc.). The position may require working evenings, weekends, or on-call coverage.

• At least 1 year of experience working with children or adolescents.
• Valid driver’s license with a clean driving record.
• Ability to work Monday to Friday with an on-call rotation.
• Bachelor’s degree.
