Hospital Pharmacist
Position Summary:
The Hospital Pharmacist is responsible for providing safe, effective, and evidence-based pharmaceutical care within a hospital setting. This role collaborates closely with physicians, nurses, and other healthcare professionals to ensure optimal therapeutic outcomes for patients. The ideal candidate will possess strong clinical knowledge, sound judgment, and a commitment to patient safety and continuous improvement in pharmacy practice.
- Review, interpret, and verify physician orders for accuracy and appropriateness.
- Provide clinical consultations on medication selection, dosing, interactions, and side effects.
- Monitor patient therapy outcomes and adjust medication plans as necessary.
- Participate in clinical rounds, emergency response teams, and critical care consults as needed.
- Ensure compliance with hospital policies, Joint Commission standards, and applicable state and federal laws.
- Oversee preparation, compounding, and dispensing of medications following established protocols.
- Maintain accurate records of controlled substances and ensure proper storage and handling.
- Collaborate with interdisciplinary teams to optimize medication use and minimize adverse events.
- Provide guidance and education to pharmacy technicians and other support staff.
- Participate in continuing education and professional development programs.
- Contribute to pharmacy staff training and hospital education initiatives.
- Engage in quality improvement projects and research when applicable.
- Education:
Bachelor's Degree in Pharmacy (PharmD preferred). - Experience:
Minimum 2 years of hospital pharmacy experience. - Training:
Completion of an accredited clinical pharmacy training program (residency preferred). - Certification:
Board of Pharmacy Specialties (BPS) Certification required. - Certificate in Emergency Medicine or Critical Care required.
- Licensure:
Active and unrestricted state pharmacist license in good standing.
- In-depth knowledge of pharmacotherapy, sterile compounding, and medication safety.
- Strong communication and interdisciplinary collaboration skills.
- Ability to prioritize and manage multiple clinical demands in a fast-paced environment.
- Commitment to continuous learning and professional excellence.
