• Visit 5–6 patients daily and provide individualized physical therapy in the home.
• Evaluate patient needs and develop treatment plans.
• Educate patients and caregivers on exercises and care strategies.
• Document care and progress using Homecare Home Base.

• Delaware Physical Therapist license (Compact PT accepted).
• Home‑health experience preferred; experience in SNF or ALF settings with geriatric patients preferred.
• At least 1 reference from entire work history.
