Job Overview:

As a Behavioral Health Case Manager, you will provide professional social work services in a psychiatric outpatient setting. Your role involves overseeing and participating in comprehensive patient care services, including assessments, treatment planning, direct service provision, person-centered services, and management of client risk and safety. You will also collaborate with other state and private agencies and refer clients to appropriate social service agencies.

Requirements:

• Possession of a Master's Degree in Behavioral or Social Science or a related field.
• Three years of experience in case management, including assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet an individual's human service needs.
• Three years of experience in providing clinical treatment, counseling, or determining eligibility for services or benefits as part of a treatment team.

Preferences:

• Experience in a psychiatric outpatient setting.
• Strong communication and interpersonal skills.
