Job Overview:

As a Behavioral Health Case Manager, you will provide professional social work services in a psychiatric outpatient setting. Your role involves overseeing and participating in comprehensive patient care services, including assessments, treatment planning, direct service provision, person-centered services, and management of client risk and safety. You will also collaborate with other state and private agencies and refer clients to appropriate social service agencies.

Requirements:

Possession of a Master's Degree in Behavioral or Social Science or a related field.
Three years of experience in case management, including assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet an individual's human service needs.
Three years of experience in providing clinical treatment, counseling, or determining eligibility for services or benefits as part of a treatment team.

Preferences:

Experience in a psychiatric outpatient setting.
Strong communication and interpersonal skills.
