    Essential Functions:

    • Learn and practice the Principles of Everyday Lives when offering support to our individuals.
    • Embrace the Charting the Life Course assessment and create a life-map to help steer decision making and set individual goals.
    • Ensure that the individual is involved or always considered in decision-making and that choice is expected, not optional.
    • Explain each individuals' Rights and the responsibility and risk those Rights carry until comprehension is achieved.
    • Create a relationship/connection with the individual so that you can comfortably discuss each person as an individual, not a case.
    • Determine who the significant stakeholders are in the individuals life and establish a healthy rapport with them.

    Program Knowledge, Skills and Responsibilities:

    • Oversee the coordination of services for the individual and promote continuity and consistency in their service delivery
    • Comply with State mandated regulatory Program Specialist (known as Personal Outcomes Champions at Ken Crest) requirements for all programs that touch the lives of the individuals that are presented on your caseload.
    • Practice and encourage professional and healthy communication with all persons who have an influence on the individuals on your caseload.
    • Ensure that the individual's medical, dental, social, financial, behavioral, spiritual and personal needs, wants and desires are being accounted for and addressed.
    • Emergent situations, upon discovery or notification, will be prioritized. Necessary and appropriate actions should also be considered a priority and communicated as such.

    Data and Analytics:

    • Collect and report data required by department KPIs.
    • Analyze current data to identify areas of improvement and opportunity.
    • Provide suggestions to supervisor on ways to improve outcomes
    • Participate in Ken Crest initiatives as appropriate to contribute to organizational success.

    Additional

    Job Responsibilities:

    • Actively implement the Standards of Practices established for the department
    • Participate in both required and optional professional trainings
    • HIPAA compliance will be maintained at all times, including at the mobile and touchdown work spaces
    • Maintain familiar and working knowledge of both regulatory and Ken Crest expectations
    • Seek direction and/or support from colleagues or Supervisors as necessary
    • Attend department meetings as scheduled (in person or remotely)

    Minimum Qualifications:

    • Bachelor's degree in Human Services or related field from an accredited college or university and 2 years work experience working directly with individuals with intellectual disabilities preferred.
    • Clear Criminal History and child abuse check in accordance with State regulatory requirements
    • Valid Driver's license
    • Reliable Transportation – this position requires that you drive yourself from location to location daily.
    • Technological aptitude will be required, including the use of a laptop with programs such as Microsoft Outlook, Word, Evolv) and a cell phone
