Horizon House is Pennsylvania’s oldest psychiatric rehabilitation agency. Since 1952, Horizon House has been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to assist them to live as independently as possible in a community-based setting.

We are expanding our team and are seeking qualified candidates to join our Residential Services Group Home team in Sussex County, Delaware State
.
The position is scheduled for 40 hours per week, consisting of 8-hour shifts. Work schedules are developed based on program and operational needs.
- Preferred availability to work every other weekend (Saturday and Sunday) on a rotating schedule. During the alternate weeks when not scheduled to work the weekend, assigned two additional weekday shifts to maintain a consistent 40 hours per week.
- Shifts:
8:00 AM - 4:00 PM, 4:00 PM - 12:00 AM and 12:00 AM - 8:00 AM - Facilitate and reinforce skill development in social supports, in areas including food management, self-care preservation/personal hygiene, relapse prevention, medication management, self-care preservation/personal hygiene, money management money, and planned community activities.
- Complete documentation, including progress notes and service plans, as required by regulations.
- Establish an ongoing dialogue with other members of the treatment team, including program staff, vocational/educational instructors, consultants, members of the medical profession, and family members to provide continuous support.
- Perform other duties and responsibilities as they relate to the overall enhancement of program service delivery.
- High school diploma or GED required, 2 years’ experience working in the mental health field, or any combination of experience, education, or training that would provide the level of knowledge, skill, and ability preferred.
- Very good communication skills and detail-oriented.
- Ability to work independently and as part of a team.
- Proficiency in various computer technologies including Electronic Health Record, Outlook, MS Word, and Excel.
- Valid Driver’s License with acceptable driving record.
- Requires the ability to lift, carry, fold, kneel, stack, stoop and bend which may include direct intervention with
participants - Frequent talking and listening.
- Possess manual dexterity and fine motor skills.
- Requires crisis intervention in emergency situations.
- May be required to report to work during emergencies, including inclement weather.
