Job Summary

Signature Rehab is seeking a qualified Physical Therapist to provide rehabilitative care to residents and patients under an integrated care model.

$8,000 Sign‑On Bonus available. This role works closely with interdisciplinary clinicians to promote full recovery and timely return to daily activities.

• Work as a rehabilitative therapist under the integrated care model, collaborating with other clinical caregivers to ensure residents and patients achieve full recovery and timely return to desired activities.
• Assess and set patient recovery goals and develop individualized therapeutic plans.
• Educate patients and families on exercises, mobility, and preventive strategies.
• Maintain accurate and timely documentation of progress and interventions.

• Graduate of an accredited physical therapy program, culminating in a minimum of a Bachelor’s of Science Degree.
• Valid Physical Therapy license in the state where services are rendered.
• Strong communication and teamwork skills.
• Experience in a nursing home or rehabilitation setting preferred.
