Bayhealth Medical Center, Kent Campus, is seeking a Transitions of Care Clinical Pharmacist to ensure safe, effective movement of patients from hospital stay to discharge. You will coordinate with physicians, nurses, social workers, and care managers to reconcile medications and optimize pharmacotherapy.
You will partner with inpatient and community pharmacy services to coordinate orders, delivery, and patient counseling at discharge.
