General SummaryThe Population Health Pharmacist is a licensed pharmacist that will provide support for clinical pharmacy services to participating health plans. Responsibilities will be focused on improving the health of Delaware residents by improving quality of pharmacy-related care (STAR measures) and cost containment strategies through provider collaboration with providers engaged in value‑based arrangements Population Health Pharmacist is responsible for providing clinical pharmacy support to Medicare and Commercial providers.
Responsibilities- Education:
Bachelor Degree Pharmacy - Credential(s):
Licensed Pharmacist, Board of Pharmacy Specialties (BPS) Certified in a specialty, Board of Pharmacy Specialties (BPS) Certified in Pharmacotherapy, Ambulatory Care, Cardiology, Geriatrics, or related area. - Experience:
Required:
One year of hospital pharmacist experience as a clinical pharmacist or completion of ASHP accredited PGY-1 residency program. Successful completion of Bayhealth credentialing process if applicable.
- Education:
Doctorate Degree Pharmacy - Credential(s):
Board Certification with one year of clinical pharmacy experience or PharmD with Board Certification in Pharmacotherapy or advanced degree (ex. MPH or MBA) - Experience:
Preferred:
Two years of hospital pharmacist experience as a clinical pharmacist or completion of ASHP accredited PGY-2 residency ambulatory program.
