Case Manager
Under the supervision of the Resident Manager, the Case Manager is responsible for ensuring adherence to program operational procedures while utilizing Psychiatric Rehabilitation (Psych Rehab) principles to support recovery, community integration, and improved quality of life for residents. The Case Manager works collaboratively with residents, family members, team members, community partners, and other service providers to develop and support individualized recovery goals.
This position promotes a safe, supportive, and recovery-oriented environment that fosters independence, self-advocacy, skill development, and community inclusion. The Case Manager is expected to model and support the values, mission, and policies of the organization while assisting staff and stakeholders in understanding and implementing Psychiatric Rehabilitation practices.
Recovery and Rehabilitation Services- Utilize Psychiatric Rehabilitation principles to promote recovery, community integration, self-determination, and improved quality of life for residents.
- Support residents in identifying, pursuing, and achieving individualized recovery goals.
- Assist residents in maintaining and improving their physical, emotional, behavioral, and mental well-being.
- Encourage active participation in the development and implementation of individualized recovery plans.
- Promote resident independence, empowerment, and personal growth through person-centered services.
- Foster the development of natural supports and community connections.
- Develop positive, professional, and supportive relationships with residents.
- Provide emotional support and guidance while maintaining appropriate professional boundaries.
- Assist residents in developing skills necessary for independent living and successful community participation.
- Encourage and support self-advocacy and resident involvement in decision-making processes.
- Collaborate with residents, families, treatment teams, and community providers to address service needs and support recovery outcomes.
- Educate and support residents in developing daily Independent Activities of Daily Living (IADLs), including:
- Household management
- Meal preparation
- Personal organization
- Budgeting and money management
- Community navigation
- Appointment management
- Provide coaching and support to increase residents' ability to function independently in community settings.
- Maintain a safe, supportive, and recovery-focused residential environment.
- Assist in creating a collaborative and welcoming atmosphere within the home.
- Perform light housekeeping duties as needed to support a clean and safe living environment.
- Monitor living conditions and promptly report safety, maintenance, or environmental concerns.
- Support daily program operations and ensure compliance with organizational procedures.
- Promote resident access to community resources, supports, services, and opportunities.
- Encourage participation in social, vocational, educational, and recreational activities.
- Practice and encourage advocacy efforts that empower residents to advocate for their own needs and goals.
- Facilitate connections to community-based supports that enhance recovery and independence.
- Maintain accurate, timely, and confidential documentation in accordance with organizational requirements and applicable regulations.
- Participate in team meetings, case reviews, and treatment planning activities.
- Communicate effectively with residents, families, team members, and external service providers.
- Provide feedback and support to team members regarding the implementation of Psychiatric Rehabilitation practices.
- Perform additional duties and responsibilities as assigned by the Resident Manager or other designated leadership staff.
- Support organizational initiatives and participate in required training and professional development activities.
- Bachelor's Degree in a Human Services field, including but not limited to:
- Social Work
- Psychology
- Human Services
- Counseling
- Sociology
- Behavioral Health
- Related disciplines
- Valid Driver's License with an acceptable driving record.
- Ability to work effectively with individuals experiencing mental health challenges and recovery needs.
- Strong interpersonal, communication, and organizational skills.
- Commitment to recovery-oriented, person-centered practices.
- Understanding of Psychiatric Rehabilitation principles and recovery-oriented service delivery.
- Ability to establish trusting, professional relationships with residents and stakeholders.
- Ability to support resident self-advocacy, choice, and empowerment.
- Effective verbal and written communication with residents, families, providers, and team members.
