SUMMARY

The Community Health Worker Liaison is a frontline public health worker who is a trusted member of and/or has an unusually close understanding of the community served. This trusting relationship enables the worker to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to social determinant of health services and improve the quality and cultural competence of service delivery.

The Community Health Worker Liaison also builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support, and advocacy.

As a member of the Women's Health Screening Team, the Community Health Worker Liaison will be responsible for identifying social, emotional, environmental, financial, and other support needs for community members and established constituents while connecting them with appropriate services and community resources. This position will work closely with our Women's Health Screening and Survivorship teams and community partners to identify areas of need and resources with the goal of providing breast screenings and improving patients' health care and outcomes.

MINIMUM

QUALIFICATIONS
  • BA degree preferred or equivalent work experience in education, health-related field, social sciences, or social work.
  • Bilingual – Haitian-Creole language oral and written communication required.
  • Community Health Worker Certification (preferred).
  • Proven community development skills and knowledge of working with diverse communities.
  • Knowledge of working with diverse communities.
  • Demonstrated competence in Microsoft Office programs.
  • Excellent interpersonal, writing, collaboration, and problem-solving skills. Must be a team-player.
  • Valid driver’s license and excellent driving record required.
KNOWLEDGE OF
  • Key components of cultural competency and awareness of differences, attitudes, and beliefs that would allow one to work across cultures effectively and appropriately.
  • Social determinants of health and related disparities.
  • Health Issues, conditions, and cultural health practices of communities served.
  • Healthy lifestyles and self-care.
  • Mental/behavioral health issues and their connection to physical health.
  • Health behavior theories, basic public health principles, the community served, United States health and social service systems, community/public health services.

Ability to write reports, business correspondence, and standard operating procedures (SOPs).

Ability to effectively present information and respond to questions from clients, partners, staff, and the public.

REASONING ABILITY

Ability to solve practical problems.

Ability to interpret a variety of instructions provided in written, oral, or schedule format.

ESSENTIAL POSITION REQUIREMENTS
  • Traveling between offices and communities in Delaware to conduct meetings and outreach events with constituents in need of breast health resources to provide case management by completing needs assessment, identifying social needs, developing effective service plans, collaborating with healthcare systems, coordinating care and connecting to appropriate internal and/or external community resources.
  • Assist individuals and families in the community in accessing health and psycho-social related services, including but not limited to obtaining a medical home, providing instructions on appropriate use of the medical home, overcoming barriers to obtaining needed medical care i.e. clinical breast exams, behavioral health and social services while reducing cultural and socio-economic barriers between…
