PRN/Per Diem Social Worker

Prestige Healthcare is currently hiring for a PRN/Per Diem Social Worker for the DE market.

Social Worker experience in long term care is highly preferred.

Job Duties and Responsibilities of the Social Worker:

• Demonstrates knowledge of age specific developmental factors specific to adult and geriatric residents (i.e. physical, cognitive, and socialization factors) in planning social services programs.
• Demonstrates knowledge, skills, and techniques necessary to plan social services programs for residents with the following needs: therapeutic, neurological and dementia, dialysis, IV therapy, and infectious diseases if applicable.
• Promptly follows-up on admission inquiries, evaluates and assesses the needs of prospective residents to determine appropriateness of facility placement
