Home Health Occupational Therapist Position

You will develop and implement individualized plans of care that emphasize ADL and IADL training, energy conservation, cognitive strategies, and caregiver education tailored to each patient's goals and living environment.

You will collaborate closely with a supportive interdisciplinary team that may include physical therapists, speech therapists, nurses, social workers, and home health aides. Communication is primarily through EMR and regular case conferences, allowing for thorough coordination of care while preserving your autonomy in the field. Strong documentation skills and efficient use of EMR systems will be important, as you will complete evaluations, progress notes, and discharge summaries electronically, often at or near the point of care.

Patient volume and visit frequency are structured to allow time for individualized, high-quality interventions. Visit schedules typically include a mix of start-of-care evaluations, routine follow-ups, reassessments, and discharges. The number of visits per day will depend on patient needs, acuity, and travel distances within the Dover area, with careful planning to optimize route efficiency and minimize windshield time. You can expect regular daytime hours with some flexibility in scheduling to accommodate patient availability, with opportunities for occasional evenings or weekends as required for continuity of care.
