An Occupational Therapist is responsible for providing specialized therapeutic services to patients in the home health setting. The role involves conducting assessments, developing treatment plans, and working to improve patients’ functional abilities.

Responsibilities:

• Evaluate patients’ physical, emotional, and cognitive abilities to determine appropriate treatment plans.
• Develop and implement customized therapy programs to meet individual patient needs.
• Document patient progress, treatment activities, and outcomes accurately within mandated time frames.
• Collaborate with healthcare teams to coordinate care and modify therapy approaches as needed.
• Travel to patients’ homes up to 75 miles from the assigned clinic to deliver therapy and monitor progress.
• Maintain current licensure, CPR, and BLS certifications, and ensure compliance with agency and state regulations.

Qualifications:

• Hold an active license as an Occupational Therapist with proof before assignment start.
• Possess a minimum of 1 year of recent adult home health experience.
• Demonstrate proficiency in Home Care Home Base (HCHB) EMR system within the past 6 months.
• Be willing to travel and have reliable transportation, a valid driver's license, and liability insurance.
• Be able to accept 5-6 patients per day, providing comprehensive treatment during home visits.
• Comfortable with documentation, travel, and working independently in patients’ homes.
