General

Summary:

The Diabetes Quality Coordinator (QC), as defined in the American Diabetes Association (ADA) Standards, is responsible for overseeing the day-to-day operations of the Diabetes Self-Management Education and Support (DSMES) service at all sites. The QC ensures that the National Standards Diabetes Self-Management Education and Support (NSDSMES) are met and maintained at all times.

Responsibilities:

1. The QC is responsible for the monitoring, facilitating, and implementing of the American Diabetes Association Education Recognition Program (ADA ERP) in accordance with the ADA ERP Review Criteria & Indicators current edition. Responsible for the ongoing evaluation of all Diabetes Wellness Center (DWC) to enhance quality of the DSMES and address gaps in service. Maintains the quality improvement process and plan for evaluating the education process and service outcomes.

Maintains the required documentation according to ADA ERP Recognitions Standards, ADA Standards of Medical Care in Diabetes and CMS Guidelines.

2. Coordinates and implements processes to assure compliance with regulatory and accreditation standards as defined within Bayhealth and by Best Practices (CMS, Joint Commission, ADA, American Association of Diabetes Educators (AADE), State of Delaware Department of Licensure)

3. Collaborates with clinicians and leadership to integrate office processes and informatics with the care and education specialists (including training, scheduling, program development, security, and other necessary functions).

4. Collaborates with clinicians and leadership to include (but not limited to), hiring, orienting, and program development, with the integration of appropriate clinical services.

5. Collaborates with clinicians and leadership to direct and monitor appropriate utilization of resources in accordance with capital and operational budget initiatives.

6. Assists Manager and staff as needed.

7. Provides DSMES for participants aged 18 and older in compliance with ADA ERP guidelines, including intensive insulin therapy and management of diabetes during pregnancy and diabetes resulting from treatment of another disease.

8. Participates in community events for community education and program visibility.

9. Organized stakeholders to provide purposeful input and/or advocacy via planning activities, preparation, meeting attendance.

10. Other duties as assigned within the scope and practice of the diabetes care and education specialist discipline.

Required Education, Credential(s) and

Experience:

  • Education:

    Bachelor Degree;
    Nursing;
    Or Bachelor's Degree in Dietetics/Dietitian.
  • Credential(s):
    Registered Nurse Basic Life Support Certified Diabetes Care and Education Specialist Registered Dietitian Nutritionist ;
    If BSN is held, valid RN licensure is required. Registered Dietician credentials are required if a Bachelor's Degree is a specialization in Dietetics/Dietician.
  • Experience:

    Required:

    Five (5) years of professional practice as an RN, RD, or Pharmacist, including 2 years as a Clinical Diabetes Educator. Experience with the ADA accreditation process.
