General
Summary:
The Dietitian provides nutritional assessment, planning, implementation, education and evaluation services designed to meet the needs of patients, family or community affected by diabetes. The Dietitian educates and coordinates activities as a member of the health care team and maintains skills and current knowledge in nutritional care.
Responsibilities:
1. Assess nutritional status and plans the nutritional care of patients who are on therapeutic diets and/or at nutritional risk as designated by regulatory agencies and approved departmental policies.
2. Communicate appropriate nutritional information through the medical record in accordance with the guidelines set forth by the Medical Records Department.
3. Implement nutrition care plan for individual patients and communicates appropriate information/recommendations via the medical record, interdisciplinary care meetings, and nutrition department.
4. Maintain appropriate records including Quality Improvement data collection records and reports within the appropriate time frame. Collaborates with patient, family and healthcare team to formulate patients' care in relation to diabetes management. Consults with other healthcare providers in determining annual focus & goals for the patient.
5. Evaluate effects of nutrition care intervention on individual patient's nutritional status.
6. Provide nutrition education for patients in a manner appropriate to the individuals' lifestyle, economic situation, food preferences, cultural preferences and age. 7. Provide plan for continuing care of patients after discharge from facility, which may include further consultation with patient or family members or consultation with community agencies.
8. Provide educational experiences for general public or outpatients.
9. Performs other duties as assigned, within the scope and range of job responsibilities.
Required Education, Credential(s) and
Experience:

Bachelor Degree;
Related field - Credential(s):
Certified Diabetes Educator - 2 yrs if not already obtained. Registered Dietitian Nutritionist Basic Life Support;
Unless already obtained, certification must be granted within 2years of date of hire. - Experience:
Required:
Three (3) years' experience in a hospital setting.
