• Contract Length: 13 weeks
• Candidate Type: Local and traveler friendly, Local candidates only - travel stipend not available, Traveler friendly - travel reimbursement available
• Requirements: Years of experience as a PTA, Qualified applicants MUST have at least Years of experience in Home Health, Valid Physical Therapy Assistant license, Be willing to obtain STATE licensure

• Deliver one-on-one, high-touch care to patients in their homes
• Conduct skilled nursing visits and perform assessments
• Create and manage individualized care plans
• Collaborate with families and interdisciplinary providers to improve patient outcomes
