Therapist – Physical Therapist

Job Summary:

Provide outpatient physical therapy services in a home health setting, focusing on adult patients through home visits. Ensure the delivery of high-quality patient care while adhering to established clinical protocols and documentation requirements.

Responsibilities:

• Conduct initial evaluations and follow-up visits for adult home health patients.
• Develop and implement individualized treatment plans based on patient assessments.
• Document patient progress, treatment interventions, and outcomes accurately within required time frames.
• Travel to patients' homes up to 75 miles from the clinic, ensuring timely and efficient service delivery.
• Collaborate with team members and attend periodic office meetings to ensure continuity of care.
• Adhere to all agency policies, including maintaining current licensure, liability insurance, CPR, and BLS certifications.

Qualifications:

• Hold a valid state license to practice as a Physical Therapist in the assigned territory.
• Possess a minimum of 1 year of recent adult home health experience.
• Be proficient in Home Care Home Base (HCHB) EMR system, with recent usage or retraining completed.
• Have reliable transportation, a valid driver’s license, and auto insurance.
• Demonstrate knowledge of OASIS documentation if applying as an RN or therapist involved in HCHB.
• Be willing to travel up to 75 miles from the home clinic location.
