Physical Therapist In Home Health
Join our team as a Physical Therapist in Home Health, where you'll provide essential care to patients in their homes.
What You'll Be Doing- Conduct home visits for patient assessments and therapy
- Document patient progress and treatment plans
- Travel up to 75 miles from the home clinic for patient visits
- Participate in periodic office meetings
- Manage a caseload of 5-10 patients per day
- Active Physical Therapist license
- CPR and BLS certification
- Minimum of 1 year of recent adult home health experience
- Reliable transportation with a current driver's license and agency-required liability insurance
- Experience with HCHB EMR system
