Licensed Practical Nurse (LPN) – Home Care, Visits. Interim Healthcare of Delaware

Interim Healthcare is seeking Licensed Practical Nurses (LPN) for home care visits throughout the State of Delaware. We have multiple opportunities for nurses to make supplemental income doing routine nursing care visits. This entails seeing a client in their home or group home setting and performing a nursing visit following specific MD orders. Daily visits include diabetes oversight (checking blood sugar and administering insulin if ordered), foley catheter changes, bolus tube feedings, wound care, trach care, medication administration, weekly medication box fills, and refills as needed.

Some visits are time‑specific and others flexible; we can work with you on building a schedule.

Essential Functions of Licensed Practical Nurse (LPN):

• Counsels and educates the patient and family in meeting healthcare and related needs.
• Completes timely and accurate clinical notes, including addressing the patient’s progress.
• Communicates with the physician and other staff regarding changes in the patient’s needs.
• Able to perform CPR.
• Must be able to lift 50 lbs.

Requirements of Licensed Practical Nurse (LPN):

• Appropriate license to practice in the State of Delaware.
• At least six (6) months experience within the last five (5) years working in a hospital, home care or nursing home setting within the United States.
• CPR certified.
• Negative TB test result.
• Pass a background check.
• Provide two (2) professional references.
• Able to travel to cases as assigned.
• Ability to work independently and manage time effectively.
• Meets the requirements of federal and/or state criminal and abuse background checks.
