In-Home Nurse Practitioner or Physician Assistant - Wilmington

Signify Health, part of CVS Health®, is seeking a full‑time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations and bridge gaps in care.

The visit lasts about 45 minutes, allowing time to connect one‑on‑one. It includes a medication and medical history review, a physical evaluation, and, if ordered by the health plan, additional diagnostic tests such as diabetic eye exams and spirometry. The clinician uses an iPad and an intuitive clinical workflow for each evaluation and is not required to prescribe medicine, order lab tests or alter patients’ current treatment regimen.

• Provide in‑home health evaluations using an iPad and an intuitive clinical workflow.
• Engage with patients in their homes, answering health questions and building trusted relationships.
• Conduct virtual visits as needed.
• Provide all Signify Health services indicated, including Diagnostic & Preventive Services products such as spirometry, diabetic eye exams, bone density screening, etc.
• Travel locally and within licensed states as required.

As an in‑home clinician, your primary workplace is the private residence of health plan members. The setting may contain household pets, secondhand smoke, and other potential allergens or irritants; candidates must be able to safely perform duties in such environments.

Pay Range: $87,035.00—$. The actual base salary will depend on experience, education, geography and other factors.
