• Administer medications: give oral, topical, IV, injectable medications
• Wound and skin care: perform wound care/prevention, manage pain, apply dressings
• Monitoring: observe physical and mental health status, track changes and analyze data
• Clinical procedures: provide catheter/ostomy care, gather specimens, etc.
• Care planning: assist in executing individualized care
• Collaboration:
  
  strategize care with physicians, therapists, social workers and others
• Family & resident education: explain health status, changes and expectations for care
• Documentation: keep detailed records of health status, all care, progress and interactions
• Team oversight: oversee and delegate CNAs ensuring quality care and teamwork

• Licensed to work within the State of Delaware as an LPN
• Pass background check, drug screen, professional references and physical evaluation
• Professional and clinically skilled with leadership qualities
