Position: NURSE - SALARY

Registered Nurse (RN) – Assertive Community Treatment (ACT) Team

The Registered Nurse (RN) – Assertive Community Treatment (ACT) Team plays a vital role in delivering comprehensive, recovery-oriented nursing services to individuals living with severe and persistent mental illness and co-occurring disorders. Working as part of a multidisciplinary ACT team, the RN provides clinical assessments, medication management, health education, crisis intervention, and care coordination services that promote stability, wellness, independence, and successful community living.

This position combines psychiatric nursing, physical health care, and community-based support to address the unique and complex needs of clients in their homes and throughout the community. The RN serves as both a clinical resource and advocate, ensuring clients receive integrated, person-centered care that supports their recovery journey.

Key Responsibilities Direct Clinical Care
  • Administer and monitor prescribed medications, treatments, and injections.
  • Conduct psychiatric, nursing, and physical health assessments.
  • Monitor client health status and identify changes requiring intervention.
  • Complete vital signs, metabolic screenings, and other ordered clinical procedures.
  • Provide direct nursing care in community and home-based settings.
  • Deliver in-home support services, including:
    • Activities of Daily Living (ADL) training
    • Medication education and management
    • Home management and wellness support
  • Educate clients regarding symptoms, medications, treatment plans, and disease prevention strategies.
  • Provide supportive counseling and health coaching to promote recovery and self-management.
  • Support medication adherence and wellness-focused treatment goals.
Care Coordination and Treatment Planning
  • Participate actively in interdisciplinary treatment planning and case conferences.
  • Coordinate medical, psychiatric, and behavioral healthcare services.
  • Ensure follow-through with medical appointments, treatment recommendations, and prescribed care.
  • Communicate significant changes in client conditions to treatment team members and healthcare providers.
  • Refer and connect clients to community-based services and external providers.
  • Coordinate access to housing, employment supports, and other community resources.
  • Maintain collaborative relationships with pharmacies, physicians, hospitals, and ancillary service providers.
  • Provide consultation and support to families, caregivers, and natural support systems.
Crisis Intervention and Advocacy
  • Respond to psychiatric, medical, and behavioral health crises.
  • Coordinate emergency services and crisis intervention efforts as needed.
  • Participate in on-call responsibilities as assigned.
  • Advocate for clients' rights, needs, and access to resources within the community.
  • Support clients in overcoming barriers to treatment and community integration.
Compliance, Documentation, and Quality Assurance
  • Maintain accurate, timely, and comprehensive clinical documentation.
  • Ensure compliance with nursing standards of practice, licensing requirements, and medication regulations.
  • Monitor medication inventory, storage, administration records, and documentation.
  • Ensure compliance with infectious disease control protocols and related requirements.
  • Train and supervise unlicensed staff in medication administration procedures, health monitoring, and care standards.
  • Participate in agency meetings, trainings, quality improvement initiatives, and professional development activities.
  • Protect client confidentiality and maintain HIPAA compliance at all times.
Required Qualifications Education
  • Graduate of an accredited School of Nursing.
  • Bachelor of Science in Nursing (BSN) preferred.
Licensure and Certifications
  • Current and unrestricted Registered Nurse (RN) license in the State of Delaware.
  • Current CPR certification.
  • Successful completion of a pre-employment Tuberculin Skin Test.
Additional Requirements
  • Ability to obtain and maintain security clearance through the Delaware Department of Correction.
  • Successful completion of all required background investigations.
  • Valid driver's license, current vehicle registration, and access to a reliable vehicle.
Preferred Experience
  • Minimum one (1) year of clinical nursing experience preferred.
  • Experience working in community mental health, behavioral health, psychiatric nursing, ACT programs, crisis services, or integrated healthcare settings preferred.
