The Assertive Community Treatment (ACT) Team provides comprehensive, community-based behavioral health services to individuals with serious mental illness and co-occurring disorders. Our team works collaboratively to ensure clients receive the support, treatment, and resources they need to successfully live, work, and thrive in their communities.
As an ACT Team RNYou will play a vital role in coordinating healthcare services, managing medications, supporting wellness, and advocating for individuals throughout their recovery journey.
What You’ll DoAs a Registered Nurse on the ACT Team, you will:
Clinical Nursing Care- Conduct comprehensive psychiatric, clinical, and physical health assessments
- Monitor client health status and treatment progress
- Administer medications, injections, and prescribed treatments
- Monitor medication effectiveness, side effects, and treatment outcomes
- Maintain medication records and ensure regulatory compliance
- Complete nursing interventions, screenings, and health monitoring activities
- Provide nursing services in homes and community settings
- Support medication self-management and independent living skills
- Promote wellness, disease prevention, and health education
- Deliver individualized interventions that support recovery and self-sufficiency
- Coordinate services with physicians, psychiatrists, specialists, pharmacies, and hospitals
- Assist clients with accessing healthcare and community resources
- Facilitate referrals and follow-up care
- Build strong partnerships with healthcare providers and support systems
- Respond to psychiatric, behavioral health, and medical crises
- Participate in interdisciplinary treatment planning meetings
- Collaborate with psychiatrists, therapists, case managers, and peer specialists
- Provide clinical recommendations and nursing expertise to the treatment team
- Complete accurate and timely clinical documentation
- Maintain compliance with nursing standards, HIPAA requirements, and agency policies
- Support infection control and quality improvement activities
- Provide clinical consultation to team members
- Support and train staff on medication administration and health monitoring
- Advocate for client rights, access to care, and community integration
