- Bachelor's degree

We are seeking a Case Manager to join our Children's Behavioral Health Program serving Clients in Kent County, Delaware. Earn $20.67/hour!
Responsibilities- Coordinate resources and information pertinent to client needs.
- Effectively and efficiently document all coordination of resources.
- Coordinate referrals for client-specific discharge planning; ensuring that the family actively participates in creating a multi-faceted discharge plan that represents the family's choice for service providers and community resources.
- Monitor record keeping, compliance and authorized units remaining, ensuring that client charts are up-to-date and include all necessary documentation, including documentation of significant incidents within 24 hours as needed (incident reports).
- Monitor/assess document safety
- Complete necessary documentation of each case, which complies with the standards set by Merakey, county MHDS/CASSP, OMHSAS, and other licensing/regulatory/payor organizations.
- Case sessions are primarily delivered in the community (client homes, schools, etc.). Case management can be in an office environment or community. May at times be required to work evenings and/or weekends to meet the needs of the clients. This position may require on-call coverage.
