Health/Human Service Case Manager II
Within this role as a Health/Human Service Case Manager II, you will determine initial and ongoing eligibility for all needs-based benefits which includes:
Food Supplement Benefits (SNAP), Medical Assistance, Delaware Healthy Children's Program (CHIP), Subsidized Childcare, State Funded General Assistance (GA), and Temporary Assistance to Needy Families (TANF). You'll also work closely with TANF and SNAP vendors. In addition, you will counsel, case manage, and make referrals for benefits DSS does not provide (such as Child Support, foreclosure assistance, or food closets). Case Managers will fulfill the diverse needs of vulnerable and low-income residents by sharing information about benefits in timely, accurate manner.
