Ambulatory​/Outpatient Pharmacist WDH

Overview

Per-Diem Ambulatory / Outpatient Pharmacist provides professional pharmacy services across both Outpatient/Community Pharmacy and Pre-Surgical Medication Reconciliation/Patient Visit areas. In this cross-trained role, you will optimize medication therapy, ensure accurate patient medication histories, and deliver safe, timely, and compassionate care to patients and their families. This role requires flexibility, collaboration, and strong communication skills, supporting both retail operations and clinical pre-admission visits as part of Wentworth-Douglass Hospital's commitment to outstanding patient care.

• Provide professional pharmacy services across outpatient, community, and pre-surgical settings.
• Optimize medication therapy and ensure accurate medication histories.
• Deliver safe, timely, and compassionate patient care.
• Collaborate with retail operations and clinical teams; participate in pre-admission visits as needed.
• Support quality improvement efforts within the department.

• Education:
  
  Doctor of Pharmacy required.
• Experience:
  
  2-3 years working as a pharmacist preferred.
• Licenses and Credentials:
  Pharmacist license in the relevant state;
  Massachusetts Controlled Substances Registration (MCSR) preferred; HR considerations apply.
• Knowledge, Skills and Abilities:
  
  Extensive knowledge of Pharmacy operations including IV Admixtures, Drug Information, Ambulatory, Manufacturing, and Drug Distribution; attention to detail; ability to assess patient data relative to age-specific needs; capability to provide care per department policies and procedures; independence in work; participation in department quality improvement and training efforts; completion of 20 continuing education credits per year to maintain license; familiarity with legal requirements, drug information and calculations, drug handling and storage practices.
