About The JobProvides professional physical therapy services to clients/patients of all ages and families whose needs change from basic to complex and are rapidly changing. Plans and implements age‑appropriate specific treatment programs for individual client/patients with a level of skill in accordance with accepted principles and practice.
What You’ll Do- Plans, develops, and implements physical therapy care plan appropriately.
- Regularly re‑evaluates and updates appropriateness of care plan and physicians’ orders.
- Communicates appropriate information to patients, physicians, peers, managers, physicians, departments, and other agencies involved.
- Integrates problem solving methodology and quality improvement in resolving problems related to patient care and organizational processes.
- Orients and supervises PTA’s according to State of NH guidelines and VNA policy.
- Education:
Degree from an American Physical Therapy Association (APTA) accredited program required. - Experience:
Minimum one (1) year of experience in health care setting performing similar work activities preferred. - Certification/Licensure:
Licensed as a Physical Therapist by the state of New Hampshire Office of Licenses Allied Health Professionals required. BLS, CPR required. - Software/Hardware:
Documentation on laptop computer, basic typing skills. - Schedule:
Participate in rotating weekend commitment. - Other:
Valid Driver’s License required to travel throughout southern New Hampshire to provide home health care service and other job‑related activities required. Proof of automobile insurance required, and minimum coverage met. Motor vehicle background report will be conducted.
