BAYADA Home Health Care is seeking a Home Health Occupational Therapist for a position in Dover, New Hampshire.

Location:

Dover, New Hampshire (Londonderry office).

Employment Type:

Staff (Full time, part time, or per diem).

• Perform outpatient therapy evaluations and develop a plan of care for patients in senior living communities or group homes for adults with intellectual disabilities.
• Operate at the top of licensure, utilizing current research‑based approaches to maximize functional independence.
• Collaborate with home health therapy partners to transition care from home health to outpatient services, ensuring client‑centered goals are met in their living environment.
• Educate clients, families, and community caregivers on care plans to build a team approach to functional independence.
• Perform evaluations, treatments, and discharge planning, documenting care in the outpatient EMR platform.

• Current New Hampshire Occupational Therapist license.
• Minimum one year recent work experience as an OT.
• Graduation from an occupational therapy program accredited by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem‑solving skills.
• Solid computer skills; prior EMR experience preferred.
• Prior home‑care experience preferred.
